2008-2009 SEASON
The Board of Directors of the Valley Figure Skating Club (VFSC) welcomes you to another season of skating. We hope this packet of information will be helpful to you as you register for the coming year. If you have any questions, please call Brenda Protheroe, Membership Chairperson at 920-729-9625
The following forms must be returned by June17, 2008 to begin or continue your membership with Valley Figure Skating Club.
_____1. SKATER INFORMATION SHEET: Please fill out all information requested on the form. This information is necessary for a complete record. PLEASE NOTE: phone numbers and email addresses that are provided are for VFSC use only. Car/cell number provided will only be used in the event of an emergency.
_____2. SKATER EMERGENCY FORM: This form is very important and must be filled out completely and accurately. A copy of this form is kept at both rinks in the case of an emergency. Both parent/guardian(s) must sign this form.
____3. AMATEUR ATHLETIC WAIVER AND RELEASE OF LIABILITY: Please read carefully, complete and return with registration material. Please fill out one form for each skater in the family.
____4. VOLUNTEER COMMITMENT/ADVERTISING
SALES/MUSIC MONITOR/FUNDRAISING COMMITMENT: All members will be required to contribute volunteer hours per calendar
year. The calendar year is July
1, 2008-June 30, 2009. There will
be many volunteer opportunities throughout the year. Volunteers are needed for ice monitor,
music monitor, help with the competition and the ice show. There are also many other opportunities
to help out, just ask a Board Member for some suggestions. It is the responsibility of the club
member to seek and arrange volunteer hours with the volunteer coordinator. The volunteer coordinator is Kendra
Weindling - (920) 731-2246. The form included in this packet needs
to be signed in both areas (volunteer and advertising sales) to be complete.
____5. SKATER’S RULES FOR MUSIC: Please go over these rules with the skater. The skater and the parent/guardian must sign this form.

____7. MEMBERSHIP DUES: When you return the ice registration materials, remember to include your club dues for the 2008-2009 season. These dues include club membership and USFS membership (including “Skating Magazine” from USFS). Keeping your dues and your ice contracts and walk-on sessions up to date will allow you to skate, test, and compete as a VFSC member and to be in the ice show. If these are not kept up to date, your forms for these events will not be signed. If you have any questions or problems concerning your bill, please call the Membership Chairperson, Brenda Protheroe @ (920) 729-9625.

submit a resume to VFSC Board prior to coaching on club ice.
Must
have personal liability insurance to be able to coach on club ice.
Allowed
6 sessions coaching on club ice as a visiting coach before membership is
required
Must have a current PSA membership.
Must complete
a background check thru PSA
Must complete
PSA & USFS continuing education credits as deemed by USFS & PSA
(Note: this goes into effect 7/1/2010, but you may start
earning credits 7/1/08) Must
participate in club events as deemed necessary by the VFSC Board
